Specific Comments Concerning the Form <br />Section I-General Information <br />1. NA <br />2. What is the project name? <br />3. Is the contact person the same as the prospector? <br />4. NA <br />5. Adequate as submitted. <br />6. Is all land ownership private? During the inspection it was indicated that some of <br />the land was located on BLM or USFS property. <br />7. If located on BLM or USFS land provide claimant and site/claim information, <br />location map, staking information, specific land management agency, and <br />documentation that appropriate notification was submitted to the land <br />management agency. If not on BLM or USFS please clearly indicate that to be <br />the case. <br />Section II-Maps and Drawings <br />An accurate topographic base map showing the proposed location must be <br />submitted with this notice. The prospector may submit a U.S.G.S. 7.5 quadrangle <br />or similar map of adequate scale that: <br />a. Identifies the proposed prospecting sites or activity areas involving surface <br />disturbance. Activity areas include all proposed or existing drill holes, <br />mud pits, excavations, trenches, adits, shafts, tunnels, rock dumps, <br />stockpiles, impoundments, prospecting roads, etc. and <br />b. Includes sufficient detail to identify and locate known prospecting features <br />and facilities that may be or already affected and those that are not <br />anticipated to be affected. This includes the location of all drill holes, <br />mud pits, excavations, trenches, adits, shafts, tunnels, rock dumps, <br />stockpiles, impoundments and prospecting roads. <br />Section III-Project Description <br />Please address each subsection in detail including: <br />1. Minerals and Resources being investigated <br />2. Estimated dates of commencement and completion <br />3. Please verify that 6 tons is the total amount of material to be moved during the life <br />of this prospecting proposal. <br />4. Will there be any drilling and blasting or other methods of prospecting other than <br />those marked?
