Distance from Utah 93 and Wray <br />Mesa Road Roads is 13 miles. Distance from Utah 93 and San Juan County Road D2797 <br />and Montrose County Colorado is 6 miles. Improvements will be dip and roll water bars, <br />grading, aggregate and culverts. No new roads. Road improvements remain within <br />existing road prism. Drainage and run-off conveyance structures were designed and <br />directed by BLM, Montrose, and San Juan County These include culverts, ditches, and <br />roll-and-dip water bars. <br />h) Water usage that is in conjunction with the operation is unchanged and is addressed in the <br />original reclamation application dated 6/11/05 Exhibit C. <br />i) Groundwater encountered or surface water intercepted: please refer to Exhibit T/ EPP of <br />this amendment application and original reclamation application dated 6/11/05 Exhibit C. <br />j) Water Laws: Refer to letter on-file with DRMS from Division of Water Resources dated <br />January 20, 2009 (attached). <br />k) Refuse and Acid or Toxic Materials; please refer to Exhibit T/ EPP of this amendment <br />application and adequacy response dated 10/07/05 (attached). <br />1) Hydrologic Balance; please refer to Exhibit T- Environmental Protection Plan of this <br />amendment application and adequacy response dated 10/07/05 (attached). <br />m) No ore processing on-site. <br />n) Commodities to be mined/ extracted; Uranium and Vanadium ores. <br />o) Explosives will be used in conjunction with mining. Explosives have been used on a <br />weekly basis from April 2008-09 with no geologic stability problems associated on or off <br />the mine permit boundary. Constructed slopes will be < 3:1. The adit entrance will be <br />stabilized using chain link fencing and engineered straps bolted to the host rock. Also <br />reference adequacy response dated 10/07/05 (attached). <br />2.
