Three (3) possible <br />problems identified. <br />• Problems and Corrective Actions: <br />PB-l: Some monitoring wells are not tightly capped and not locked. <br />All monitoring wells are now tightly capped and locked. A series of photos <br />taken August 25, 2009 are attached in Appendix I to this response. The photos <br />include the Cash Mine well area and the locking mechanisms on all wells. Bob <br />Mason of Gold Hill is the scale figure in all of the photos except for those of <br />monitoring well (MW 1) number one. These figures are Jim Smith of Gold Hill <br />and this author. <br />MRV notes that several of these wells pre-date the DRMS (then Division of <br />Minerals and Geology) permitting, with all but one of these well's pre-dating <br />MRV's operations and that to MRV's recollection, this issue has not been <br />previously identified. MRV hopes that the division recognizes that MRV <br />addressed this issue in a timely manner. <br />PB-2: The escape-way shaft has not been approved as part of the reclamation permit. <br />This is incorrect. The escape raise was driven under a prospecting Notice of <br />Intent disturbance, which was subsequently amended into the M-1983-141 <br />permit via AM02, approved by the DRMS on October 10, 2006.
