<br />Exhibit N (page 3) <br />5. disturbed areas will be monitored for noxious weeds for three (3) years following final reclamation of the <br />project's surface disturbance. <br />6. Control of noxious weeds will be required through successful vegetation establishment and/or herbicide <br />application. If applications of herbicide are prescribed, however, it is the responsibility of the permittee to <br />insure compliance with all local, state and federal laws and regulations, as well as labeling directions <br />specific to the use of any given herbicide. <br />A Pesticide Use Proposal (PUP) will be approved prior to application of herbicides and/or other pesticides <br />on Federal surface; contact the Little Snake Resource Area office to obtain a PUP form to request this <br />authorization. Submit the PUP 2 months in advance of planned application. In the event you elect to apply <br />herbicide or other pesticide as described and authorized on the approved PUP, you must report this use <br />within 24 hours on Bureau of Land Management form titled Pesticide Application Record. <br />7. All equipment and machinery shall be equipped with spark arrestors and mufflers. <br />8. Permittee is responsible for all suppression costs for any fire resulting from their operations and practices. <br />9. Topsoil will be stockpiled for pit rehabilitation measures. <br />10. All pits shall be marked as Moffat County material sites. <br />11. "No Dumping" signs will be erected at the site. <br />12. Warning signs shall be placed where appropriate during operational phases to minimize hazards to passing <br />traffic. <br />13. Between operations, the pit walls shall be maintained at a slope ratio not to exceed 3H: 1 V, so as to <br />minimize slope failure potential and public safety hazards. <br />14. Yearly production figures for each pit shall be reported on the annually to the LSFO mining engineer. <br />15. Assignment of materials from these pits for other than Moffat County purposes shall be made only after <br />notification and approval from the Bureau of Land Management, Craig District Office. <br />16. At the Area Manager's discretion areas stripped of vegetation shall be re-seeded, after disking, with the <br />following mixture (per acre): <br />Drilled Rate - PLSAbs/ac Broadcast Rate <br /> PLS/lbs/ac <br />Western Wheatgrass 5.0 lb/ac = 37% 10.0 lb/ac = 37% <br />Intermediate Wheatgrass 5.0 lb/ac = 37% 10.0 lb/ac = 37% <br />Sheep (or Hard) Fescue 3.0 lb/ac = 22% 6.0 lb/ac = 22% <br />Cicer Milkvetch 0.5 lb/ac = 04% 1.0 lb/ac = 04% <br />Exhibit N (Pqge 3)
