Inspection Report - Problem 4 <br />Evidence of erosion from the three temporary ore piles at the mill site was observed during the <br />inspection. <br />Inspection Report - Corrective Action <br />The operator must with in 30 days submit proof of erosion control for the three temporary <br />stockpiles. <br />Operator Response -- Extension request of erosion. control <br />July 23, 2009 <br />The Leadville Mill has elected to use a combination of erosion and sediment control measures <br />for the temporary stockpiles. The existing controls (natural minor depressions, constructed <br />berms, and vegetative filters) will be augmented with silt fence on the downslope sides of the <br />temporary stockpiles. The silt fence is to be located within several feet of the permit/disturbed <br />area boundary as a temporary perimeter siltation control device. <br />The permit/disturbed area boundary stakes have been reestablished around the stockpiles. Pre- <br />attached fabric silt fence with 2"x2"0' wood posts on 10 foot centers has been purchased and <br />delivered to the site. The skid steer loader with trencher attachment has not arrived on site. A <br />extension of 30 days is requested to install the fence. The Leadville Mill will supply the <br />Division with a typical "as-built" installation diagram of the silt fence and pictures of the <br />installation. <br />The silt fence installation will follow these guidelines: <br />• trench 6-8in deep <br />• silt fence fabric 6-12in placed in trench <br />• fabric buried, trench back filled and tamped <br />• shallow sediment storage depression upslope of the installed silt fence <br />• fabric NOT INSTALLED across channels, culverts, drainages
