proposed site and submit an appropriate certification to the Division in accordance with Rule <br />1.6.2(1)(b). <br />4. The mine plan map appears to be a copy of a portion of a map submitted previously for the <br />111 permit, M-2007-080. The mine plan map submitted with this 110c application, M-2009-053, <br />did not show the name of the Applicant, was not signed by the qualified person who prepared the <br />map, did not give the date of preparation, did not clearly indicate a scale and did not display a <br />north arrow. Please submit a mine plan map which satisfies the applicable requirements of Rules <br />6.2(2), 6.3.3, 6.3.5(1) and 6.3.5(2). <br />5. The application did not include a reclamation plan map. Pursuant to Rule 6.3.5(1) the <br />application must include a reclamation plan map. Please submit a reclamation plan map which <br />satisfies the applicable requirements of Rules 6.2(2), 6.3.5(1) and 6.3.5(3). <br />6. The application did not include a reclamation cost estimation. Pursuant to Rule 6.3.4(2), <br />all 110c applications must provide an estimate of the actual costs to reclaim the site. Please <br />provide the information required under Rule 6.3.4(2). <br />7. The application includes an Exhibit F, List of Other Permits and Licenses Required, which <br />lists no other permits or licenses. If there are no other permits or licenses please state such. <br />Otherwise, please provide an Exhibit F which lists the other permits and licenses as required <br />under Rule 6.3.6. <br />8. The application did not include a performance warranty. Please find enclosed a blank <br />performance warranty for your use. Copy of the warranty forms and the Construction Materials <br />Act and Rules are also available at the Division's web site at www.mininq.state.co.us. <br />Pursuant to Rule 1.4.1(8), the Applicant has 60 days from the date of this notice, by September 8, <br />2009, to submit all the necessary documents the Division needs for the application to be <br />considered filed. If, by September 8, 2009, the application has not been determined to be filed <br />the Division may deny the application and terminate the application file. <br />Once the application has been filed with the Division the 30-day decision-making time period, <br />described under Rule 1.4.2(2), may commence.
