RE: Incomplete Application for a 110c Permit, Robins Rock Pit, File No. M-2009-053.
Dear Mr. Johnson and Mr. Robins:
On June 26, 2009, the Division received the above referenced permit application. The Division
has assigned file number M-2009-053 to the application. Please include the file number in all
future correspondence regarding this application.
Preliminary review of the information submitted indicates that the application is incomplete for the
purposes of filing. The following items must be submitted prior to the Division considering the
application filed and commencement of the 30-day decision-making time period, per Rule
1.4.2(2).
1. The Division requires the Operator company name listed throughout this application match
the company name listed with the State of Colorado's Secretary of State Office (i.e., "Robins
Construction, LLLP") rather than the company name represented by the Notary Public on page
seven of the application form ("Robins Construction LLLP"), and the company name represented
on pages one and two of the application form ("Robins Construction Company LLLP."). Please
ensure that the appropriate company name is utilized throughout the application. A new
application form has been enclosed for your use. Copy of the application forms and the
Construction Materials Act and Rules are also available at the Division's web site at
www.minina.state.co.us.
2. Several lines of the application form were left blank and did not provide the required
information. Specifically, lines 1, 5, 5.1, 5.2, and 14 were not completed. Please ensure that the
application form is completed and provides all required information.
3. The certification for posting of the notice required under Rule 1.6.2(1)(b) was for a different
permit application, the Robins Pit #1. Please ensure that an appropriate notice is posted at the
