Cripple Creek & Victor Gold Mining Company <br />Cresson Project <br />Quality Assurance Monitoring and Test Results ameO <br />Load Out Bin Relocation <br />• July 7, 2009 <br />1.3.1 Daily Construction Summary Reports <br />AMEC prepared daily construction summary reports as well as photographic documentation <br />(Appendix A) throughout the project to document the key elements of construction and the CQA <br />activities that occurred. On-site AMEC CQA personnel, the Certifying Engineer and the Project <br />Engineers discussed construction activities on a regular basis. In addition, field personnel <br />prepared daily observation reports. AMEC submitted these daily reports and a daily summary <br />report to CC&V. Daily reports are not included in this report but were available for Colorado <br />Division of Reclamation, Mining and Safety (DRMS) review during the course of the project. <br />1.3.2 Weekly Construction Summary Reports <br />AMEC prepared weekly construction summary reports which were submitted to CC&V and to <br />the DRMS. Weekly Reports from the period of April 3rd 2009 through May 6th 2009 are <br />presented in Appendix C. <br />1.3.3 Project Specifications <br />All work was performed in accordance with the design plans and specifications in Amendment <br />No. 8 to Mining Permit M-1980-244 and subsequent revisions as approved by DRMS. IFC <br />Drawings are presented in the Drawings section of this report and project technical <br />specifications are presented in Appendix D. <br />1.4 Record Survey Documentation <br />Ames Construction of Aurora, Colorado provided the Record Drawings for the activities <br />associated with the LOB relocation. A Colorado Registered Land Surveyor prepared the <br />drawings. The Record Drawings prepared by Ames are presented in Appendix E. The <br />Surveyor's certificates are presented in Appendix F. Ames also provided locations for SLF <br />density tests, SLF depth checks, geomembrane panels, and destructive tests, which were used <br />by AMEC to develop test summaries and the geomembrane panel layout drawing. <br />1.5 Certification Boundary <br />Figure 2 presents the boundary of the LOB that is being certified in this report.
