A cast iron bridge plug (CIBP) shall be set at the bottom of the casing. Either 1) a cement plug <br />to surface shall be placed or 2) at minimum, the following individual plugs shall be sequentially <br />set using one or more of the approved methods described in 40 CFR 146.10: <br />PLUG NO. 1: The plug shall span the entire Dissolution Surface aquifer from the CIBP, <br />the water producing zone plus fifty (50) feet above. <br />PLUG NO. 2: The plug shall begin fifty (50) feet below the water producing zone of the <br />B-Groove aquifer, bridge the entire aquifer and extend fifty (50) feet <br />above the aquifer. <br />PLUG NO. 3: The plug shall begin fifty (50) feet below the water producing zone of the <br />A-Groove aquifer, bridge the entire aquifer and extend fifty (50) feet <br />above the aquifer. <br />PLUG NO. 4: The plug shall begin fifty (50) feet below the Perched aquifer, bridge the <br />entire aquifer and extend fifty (50) feet above the aquifer. <br />PLUG NO. 5: The plug shall be one hundred and sixty (160) feet below ground surface <br />to ground surface. <br />AREA OF REVIEW (AOR) WIT LL REQUIREMENTS <br />The IOH-C well penetrates the confining zone within a 1/4 mile radius from the IOH-I <br />injection well. At the time the Authorization For Addition Well was issued, the CBL had not <br />been completed. The authorization to inject is contingent upon submission of the. CBL and the <br />evaluation to demonstrate that the l OH-C well does not provide a conduit.for movement of fluids <br />into USDWs. <br />GENERAL PERMIT CONDITIONS <br />Reporting of Noncompliance <br />(a) Anticipated Noncompliance. The operator shall give advance notice to the <br />Director of any planned changes in the permitted facility or activity which may <br />result in noncompliance with permit requirements. <br />(b) Compliance Schedules. Reports of compliance or noncompliance with, or any <br />progress on, interim and final requirements contained in any compliance schedule <br />of this Permit shall be submitted no later than thirty (30) days following each <br />schedule date. <br />(c) Written Notice of any noncompliance which may endanger health or the <br />environment shall be reported to the Director within five (5 days of the time the <br />operator becomes aware of the noncompliance. The written notice shall contain a <br />description of the noncompliance and its cause; the period of noncompliance <br />including dates and times; if the noncompliance has not been corrected the <br />Authorization for Additional Well: UIC Area Permit C030358-00000 <br />Well: 10H-1 EPA Well ID: C030358-08414 Page 4 of 5
