Part I Page 15 of 22 <br />Permit No. COR-040000 <br />. E. REPORTING REQUIREMENTS (cont.) <br />The Annual Report will be due to the Division on or before February 15 of the following year. The exact due date for <br />the permittee's first Annual Report will be listed in their permit certification. The Division reserves the right to require <br />additional information in the report, and more frequent reports, on a case-by-case basis, as needed. <br />d. Reporting Address: A signed copy of the above report forms(s) shall be submitted to the following address: <br />Colorado Department of Public Health & Environment <br />Water Quality Control Division <br />WQCD-P-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80246-1530 <br />2. Monitorine <br />Sampling and testing of stormwater for specific parameters is not required on a routine basis under this permit. The <br />Division reserves the right to require sampling and testing, on a case-by-case basis, in the event that there is reason to <br />suspect that compliance with the SWMP is a problem, or to measure the effectiveness of the BMPs in removing pollutants <br />in the effluent. <br />If monitoring is required, the following definitions apply: <br />a. The thirty (30) day average shall be determined by the arithmetic mean of all samples collected during a thirty (30) <br />consecutive-day period. <br />• b. A grab sample, for monitoring requirements, is a single "dip and take" sample. <br />3. Reportine of Data <br />Reporting of any monitoring data gathered in compliance with Part I.D.6 or I.D.8 shall be on an annual basis, unless <br />otherwise specified by the Division. <br />Monitoring results shall be summarized for each year (January 1-December 31) and reported on Division-approved <br />discharge monitoring report forms and submitted to the Division with the Annual Report due February 15 of each year at <br />the address above. <br />4. Reportine to Municipality <br />Any permitted facility discharging to a municipal storm sewer shall provide the municipality with a copy of the permit <br />application, and/or Annual Reports, upon request. A copy of the SWMP shall also be provided to the municipality upon <br />request. <br />Sienatory Requirements <br />a. All reports and applications submitted to the Division and/or EPA shall be signed and certified for accuracy by the <br />permittee in accordance with the following criteria: <br />1) In the case of corporations, by a principal executive officer of at least the level of vice-president or his or her duly <br />authorized representative, if such representative is responsible for the overall operation of the facility from which <br />the discharge described in the form originates; <br />2) In the case of a partnership, by a general partner; <br />3) In the case of a sole proprietorship, by the proprietor; <br />4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking elected <br />official, or other duly authorized employee, if such employee is responsible for the overall operation of the facility <br />?, from which the discharge described in the permit originates.
