• All emergency phone numbers will. be conspicuously posted and also available adjacent • <br />to the project telephone. <br />• First aid kits are provided to all Supervisors, are available in most company vehicles, and <br />will be provided for use under proper supervision to crews working in isolated areas <br />away from the first aid station. <br />• Web strappings, a wool blanket, and preferably a backboard or stretcher, shall be <br />provided for each project work site. <br />• Transportation for seriously injured employees should be by designated ambulances or <br />professional rescue unit(s). When conditions require, a company vehicle may be used <br />for transportation. No one should transport a seriously injured employee alone. It is not <br />recommended that an employee requiring medical treatment drive himself/herself. <br />9.5 Injuries <br />In the event of an injury(ies), the following general procedures should be followed: <br />• Contact emergency medical personnel and provide first aid for the injured to the extent <br />that you are trained or qualified. <br />• The responsible Contractor at the scene of the incident should make notification to <br />Contractor Supervision, as deemed appropriate. <br />• The Contractor should notify an AMSQ representative as soon as practicable. This <br />notification should occur within 8 hours or the end of shift, if the circumstances do not <br />allow the notification to be immediate. <br />• Continue to render first aid until emergency medical personnel arrive or the injured is <br />transported to an emergency medical facility. <br />• The contractor will provide his representative to accompany the injured employee(s) to <br />the medical facility to provide any needed assistance to the injured and/or his family <br />members, if needed. <br />• A full investigative report shall be submitted to AMS© as soon as possible following the <br />investigation. This investigative report shall include root cause of injury incident and <br />corrective actions that will be taken by the Contractor to prevent a similar incident. <br />9.6 Accident Reporting <br />All accidents must be reported using the report form presented in Appendix A and procedures <br />presented in Appendix B. <br />0
