require a Work: Permit depending on special situations such as simultaneous operations is <br />(SIMOPS). <br />The Work Permit document shall contain the following at a minimum: <br />• When the specified work will begin (date/time) and end and/or when a new permitwill <br />be required (date/time). A formal hand-over procedure must be in place for when Work <br />Permits are issued for periods longer than one shift/tour and/or when Work Permit <br />authorization changes. <br />• Who the permit is issued to (including company and individual's name). <br />• Where the specified work will take place. <br />• Full description of what work will be performed including proposed tasks and objectives <br />and description of equipment to be used. <br />• Special considerations for safeguarding short service employees (SSE's) <br />• Description of all major hazards which could be encountered during the job, as well as, <br />documentation of appropriate controls for each hazard identified. <br />• PPE necessary for specified work that will take place. <br />• Identify specific standards/procedures/guidelines that are applicable to work that will <br />take place. <br />• Jobs involving Permit-required Confined Space Entry (PRCS); Lockout/Tagout (LOTO); <br />Excavation and Trenching; or Hot work will generally require additional documentation. <br />• Contingency plan if work does not proceed as planned. <br />• Actions to be taken in the event of an safety and health incident including appropriate <br />emergency response and notification phone numbers for WWL and contractor. <br />• Reference to all other activities that may be impacted by work which will be performed <br />(including other Work Permits) to ensure alignment and coordination. <br />• Signatures of all workers who have reviewed the Work Permit and agree to meet all the <br />operational and safety and health requirements. <br />• Final documentation and formal hand-over procedure'declaring the work has been <br />completed and the job site left with no safety and health issues or problems and ready to <br />return to service. <br />• Need to provide for the suitable display of Work Permits. <br />.7
