<br />There are no reclamation task or cost details included. Please provide a list of anticipated tasks and costs, which show
<br />acres, volumes, distances, grades, etc., as necessary.
<br />Exhibit N, Right to Enter, Rule 6.4.14
<br />Since the amendment application involves new affected lands (including BLM-managed lands for which the
<br />application notes may not be awarded a lease), please provide documentation of the operator's right to enter all
<br />affected lands in the new amended areas. Additionally, the Division cannot accept "re-assigned" rights to any lands
<br />that were negotiated between another operator and a landowner. Finally, please be aware that if any of the existing
<br />rights to enter already-permitted lands are out of date, they too must be updated and submitted.
<br />Exhibit P, Municipalities, Rule 6.4.16
<br />Durango is identified as a municipality within 2 miles of the proposed permitted areas, but no contact information is
<br />provided as required.

<br />The documentation is adequate and a corrected version need not be resubmitted, but the Division wishes to inform you
<br />that the text of the notice names "Little Fishes, LLC" at one point as the operator.
<br />Exhibit S, Significant Manmade Structures Rule 6.4.19
<br />Adequate descriptions and/or damage agreements may exist in the permit file for structures in or near the existing
<br />permitted area, but possibly not for those in or near the amended areas. Please provide all necessary documentation for
<br />the additional structures, including but not limited to that for the gas pipeline(s), gas wells, water wells, private roads,
<br />BLM roads, houses, and the highway. Please be reminded that the Division cannot accept re-assigned agreements
<br />from other permittees.
<br />Geotechnical Stability Exhibit, Rule 6.4.20
<br />Please ensure that the necessary documentation is provided for the steeply-sloped road cut involved with the new
<br />access road, as well as the drainage and fills involved with future closure of the present access road. Currently there is
