6.4.3 EXHIBIT C - Pre-mining and Mining Plan Map(s) of Affected Lands <br />1.) Pursuant to Rule 6.2.1 (2) (b), each of these maps must be signed by the registered land <br />surveyor, professional engineer or qualified person who prepared them. Please re-submit these <br />maps with the proper signatures and any correction that must be made. <br />2.) On Exhibit C (c,d) the "Topography and Affected Land" map, the elevation labels on the <br />contour lines are too small to read and the contour interval is not defined on the map. Please <br />label the contour lines and define the contour interval in such a way so the direction and rate of <br />slope can be ascertained from the maps. <br />3.) On Exhibit C (c,d) the "Topography and Affected Land" map, please label and delineate Third <br />Creek. <br />4.) There are many permanent man-made structures that appear to be contained in the area of <br />affected land and within 200 feet of the affected land. For each of these structures please show <br />what type of a structure it is and the owner's name of the structure. <br />6.4.4 EXHIBIT D - Mining Plan <br />5.) With this amendment, Ready Mixed Pit No. 1 ( M-1985-017) will be incorporated into the <br />permit area, it states in the mine plan and the legal description in Exhibit A, that this site <br />comprises approximately 38.3 acres, however in the introduction section of the application and <br />the Division's records indicate that there should only be 36.1 acres in this permit area. Please <br />explain this discrepancy. <br />6.) Please state what depth the topsoil will be stripped and what depth the overburden will be <br />stripped. Please show where the topsoil piles and overburden stockpiles will be stored for the <br />remainder of the operation on the mining plan map. These materials will need to be stripped <br />and stockpiled separately. <br />7.) Please explain how the topsoil and overburden stockpiles will be configured to reduce <br />erosion. <br />8.) Please commit to not exposing ground water until you have an approved substitute water <br />supply plan and a well permit from the Colorado Division of Water Resources. <br />9.) The mining plan map that was submitted will also need to be signed by the registered land <br />surveyor, professional engineer or qualified person who prepared it. <br />6.4.5 EXHIBIT E - Reclamation Plan <br />10.) Since Cell #2 and Phase 18 will remain an unsealed open lake, prior to the release of the <br />site, you will need to obtain a court approved augmentation plan to replace the depletions that <br />result from long-term evaporation. In order to ensure that an augmentation plan is obtained <br />prior to site release please commit to filing an application for an augmentation plan three years <br />prior to the completion of mining. <br />2
