Location map: A location map designating the facility property, discharge points and receiving waters shall be submitted. Site Sketch: A legible general sketch of the site shall be submitted, showing appurtenant facilities (buildings, ponds, diversion ditches, stockpiles, <br />etc.), stream location, numbered discharge points, sampling and flow monitoring points.
