The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when considered <br />by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />/. The Operator has exposed groundwater on the site and has failed to provide the Division DATE 5 /29 /09 <br />with a copy of the well permit and Temporary Substitute Water Supply Plan. <br />PB or PV: _PR <br />TOPIC(S): HR <br />Rule 3.1.6 requires Operators to be in compliance with applicable Colorado water laws and <br />regulations governing injury to existing water rights. The Operator shall provide the Division <br />with a copy of a well permit and Temporary Substitute Water Supply Plan from the Office of <br />the State Engineer by May 29, 2009. <br />2. V The Operator is discharging groundwater from the pit and has failed to provide the Division 5/29 / 09 <br />with a copy of a discharge permit. <br />PB or PV: PB_ <br />TOPIC(S): EIR <br />Per the requirements of Rule 3.1.6, the Operator shall provide the Division with a copy of a - <br />discharge permit from the Colorado Department of Public Health and Environment by May <br />29, 2009. <br />3. The inlet/outlet structure for the reservoir has not been included in the approved 5/29 /09 <br />Reclamation Plan or Reclamation Plan Map. <br />PB or PV: PB_ <br />TOPIC(S): RS_ <br />The Operator shall submit a Technical Revision to account for the inlet/outlet structure. The - <br />Technical Revision must be submitted on or before May 29, 2009.
