(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-9nn9-n52 <br />INSPECTION DATE: 4/15/09 INSPECTOR=S INITIALS: MAC <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety. The <br />Two Rivers Rock site is located approximately 0.5 miles north of Milliken, Colorado near the confluence of the <br />Big Thompson and Little Thompson Rivers. The Two Rivers Rock site is permitted for 48.5 acres. <br />This inspection was conducted as part of the Division's regular monitoring program. The permit sign was posted <br />at the site entrance. The boundary markers could not be identified along sections of the east and west permit <br />boundary; please see Page 3 of this report for the required corrective action. No mining was taking place at the <br />time of the inspection; the Operator was not present for the inspection. Mining is currently progressing north <br />and west from the southeast corner of the permit area. The topsoil has been segregated and stockpiled around <br />the pit, creating a visual berm. The topsoil stockpile was not vegetated, Rule 3.1.9(1) requires topsoil stockpiles <br />to have a vegetative cover if it is not replaced within a growing season; please see Page 3 of this report for the <br />required corrective action. <br />The disturbed area was traversed using a GeoXT GPS unit. The disturbed area was determined to be <br />approximately 9.16 acres. The pit contained groundwater at the time of the inspection. The Division has a <br />current copy of the Operator's Temporary Substitute Supply Plan on file. A pump was set up at the southeast <br />corner of the pit; the pump was not operational during the inspection. The pump connects to a trench line on the <br />southeast side of the pit, which flows to the Little Thompson River. The Reclamation Permit allows for a <br />portable plant for washing, screening and crushing; there is currently no portable plant on site. <br />The Division currently holds a Financial Warranty in the amount of $294,914.
