APPLICATION FORM <br />Section 12: Post mining land use selection is very important so please consider this <br />choice carefully. If you wish to change the post mining land use after the permit is issued <br />it will require a technical revision or possibly even an amendment to the permit to be <br />submitted to the Division. Based on the information and reclamation plan submitted with <br />the application it appears that Rangeland (RL) may be a more suitable choice. <br />Page 8 - Signature Page: This page must be signed and sealed by the county clerk and <br />resubmitted. <br />6.2 GENERAL REQUIREMENTS OF EXHIBITS <br />All maps, with the exception of the index map, must be signed and dated by the person <br />that prepared them and be at a scale of between one inch = 50 feet and one inch = 660 <br />feet. All maps also must contain all the standard map elements including: name of <br />applicant, scale, legend, north arrow, title and date. The site boundary must be clearly <br />marked on all maps, and in this case, the area to be amended should also be clearly <br />marked on all maps. <br />6.4 SPECIFIC EXHIBIT REQUIREMENTS -112 RECLAMATION <br />OPERATIONS <br />6.4.1 EXHIBIT A - Legal Description <br />This section is adequate as submitted. <br />6.4.2 EXHIBIT B - Index Map <br />See comments on General Requirements of Exhibits above. These comments will apply <br />to all maps submitted, so I won't restate them repeatedly in each section. <br />6.4.3 EXHIBIT C - Pre-mining and Mining Plan Maps <br />Exhibit C(a) - It appears that the label that was stuck to the map identifying the property <br />owner immediately south of the site has fallen off and been lost. Please resubmit this <br />map with the surrounding property owners information typed directly on the map. <br />6.4.4 EXHIBIT D - Mining Plan <br />Does the county intend to mine to the final 4:1 reclaimed slope, or will they mine steeper <br />slopes and then backfill to the reclaimed slopes? If the county plans to backfill, will there <br />be enough overburden to backfill steep slopes to the final 4:1 proposed in the <br />Reclamation Plan?
