Nuvemco, LLC's Monogram Mine M-1978-222UG <br />Response to Preliminary Adequacy Review <br />March 10, 2009 <br />DBMS notes that a radiological survey was completed of the proposed pennit area. The survey provides good <br />data for some of the sites that will be mined and under exploration. The Division however is requesting a <br />focused survey on the area where the office and ore bins will be constructed. This will provide a baseline and <br />average for the area which can be used as a reclamation bench mark for this area. The grid should be a <br />maximum 25 foot by 25 foot spacing and cover the entire area where ore will be handled, stored and loaded for <br />shipping. Once the statistical average is obtained it should be noted in the reclamation plan for the area. The <br />plan should be changed to reflect what measures may be necessary to obtain the average such as a cap of <br />materials, removal and disposal or other methods the applicant may feel will be affective. <br />The radiometric survey and tabulation are enclosed as Appendix Map 3 and Appendix Table 2. <br />Section 2: Please provide a trap specifically of the ore bin J office area that is equivalent to Map T-1. <br />New Map T-2 is enclosed herein. <br />Section 12: Please revise to state that once the five (5) quarters of monitoring have been completed Nuvemco <br />will consult with DRMS on modifications to the sampling plan which may include a reduction in intervals or <br />termination of the monitoring. <br />Revised as follows: <br />12. Water Quality Monitoring Plan <br />Any groundwater found in the monitoring holes constructed by Nuvemco will be sampled and <br />analyzed on a monthly basis for five (5) successive calendar quarters. After that time, Nuvemco <br />will consult with DRMS on modifications to the sampling plan which may include a reduction in <br />intervals or termination of sampling. If significant water gathers in the Monogram Pit retention <br />pond, beyond that utilized for dust mitigation, it will be sampled and analyzed. Nuvemco has <br />Storm Water Permit # COR-040241 from the Water Quality Control Division, Colorado Dept. of <br />Public Health & Environment. <br />Section 15: Please clarify that Nuvemco understands that DBMS staff reserves the right to do its own QA1QC. <br />Nuvemco must notify DBMS staff 5 working days prior to the beginning of construction of the ore bin so a pre- <br />pour inspection can be scheduled. Regardless of a DBMS inspection a post construction and as-built report <br />shall be submitted for Division review within 30 clays of completion of the project. <br />Revised as follows: <br />15. Construction Schedule Information <br />The only construction planned for the Monogram Mine is a 40 feet by 35 feet reinforced concrete <br />ore bin which is shown on Map C-1. Nuvemco recognizes the right of DRMS to conduct its own <br />QA/QC on the construction and will notify DRMS staff five working days prior to the beginning <br />of construction of the ore bin so a pre-pour inspection can be scheduled. Nuvemco will submit a <br />post construction and as-built report to DRMS within 30 days of completion of the project. <br />16
