Mine Shaft/Stope Remediation <br />• Some of the mine workings identified in Figure 1 will be remediated as a <br />result of the locations being removed following excavation to grade. All <br />mine workings, whether requiring remediation or not as determined by <br />engineering, will be checked and verified in the field during construction <br />activities. Additional areas have been found by Ames during construction, <br />however we have not seen a location survey of the new areas. <br />C. Lateral/Slot Remediation <br />• No Activities <br />III. Stormwater Management Structures <br />A. LOB Stormwater Culverts <br />• No Activities <br />B. Roadside ditch <br />• No Activities <br />C. Best Management Practices (BMPs) <br />• No Activities. Existing silt fencing structures appeared to be in good <br />condition. <br />IV. LOB Geosynthetics <br />A. LLDPE Geomembrane <br />• Poly-Flex has sent a pre-certification list for 100mi1 LLDPE for 28 rolls and <br />80mi1 LLDPE for 5 rolls. Mil Certifications were performed by TRI of <br />Texas during the manufacturing of the liner and all tests performed were in <br />Project compliance. <br />CQA ACTIVITIES: <br />SUMMARY: <br />The CQA activities performed consisted of observing, documenting and recording the construction <br />activities of the contractor. <br />Observed work activities during the week within the LOB/Delivery Road from Station 35+00 advancing <br />to Station 16+00. All work performed was cut to Waste Dump 4. Completed the drilling of the LOB Pad. <br />Blasting occurred this Thursday in the pad cut area. Continue drilling the LOB/Delivery Road cut. <br />Night shift work consisted of cut to fill from the LOB/Delivery Road, Approximate Station 2+00 to the <br />fill in the VLF from Station K4+00 to K8+00, 0-100 feet left. Intermittent SLF processing at Dump 5 due <br />to continued mechanical problems has resulted in reduced production.
