AOC%
aamed,-y
Earth & Environmental
CQA ACTIVITIES:
SUMMARY:
The CQA activities performed consisted of observing, documenting and recording the construction
activities of the contractor. Field observations within the northwest portion of the VLF included stripping
of all frozen and unsuitable materials and placement of structural fill by method spec. as outlined in the
Project Documents. In addition, observation of the drilling which began on February 5, 2009 to delineate
the extent of underground mine workings located beneath the sediment collection pond and Kitty Lane
and Thompson Shafts located adjacent to the sub-station. Drilling was completed by February 26, 2009.
Clay processing was intermittent due to mechanical problems with the clay plant located in Dump 5. Site
visits were conducted to observe the unprocessed clay prior to screening and the processed clay. On
Friday the materials processing plant located in the Arequa Process Area started production of 5-required
materials.
No work was performed on Saturday, February 28`h. Currently no work activities are planned for
Saturdays but Ames has indicated that Saturday will be used as a make up day in the event of delay or
stoppage during the current planned work periods Monday to Friday. A discussion with CC&V (Tom
Shepard) has approved additional hours for laboratory testing, if needed.
Work has started this past week on the 9775 bench located on the existing heap leach for bench widening
for the planned re-location of the existing solution piping. The bench is planned to be 30-feet wide.
I. Testing Summary
AMEC has continued the processing of clay and structural fill samples collected during the week. An
additional 11 samples were obtained from the clay processing plant at Dump 5
One sample (SF-3-R) was collected for the structural fill being placed along the existing haul road
between the 5-point intersection and the crusher area. The fill has been placed only within the limit of the
haul road to adjust road grading for run-off management. Fill placement has started this past week from
Sta. K4+00 to K8+00; 0 to 100-feet left (Sample SF-2-R represents fill from Sta. K4+00 to K8+00). All
fill has been placed per method spec. as outlined in the Project Documents. AMEC has discussed with
Ames the removal of rock in excess of 24-inches in the fill and adjacent areas that will require removal of
uncontrolled materials, frozen ground and excavation and removal of sediment retention areas. These
items were discussed with Ames and CC&V and agreed.
Sheet 4 of 8 VLF Weekly Report WE 02.27.09.D
