LLDPE Geomembrane <br />• No Activities <br />CQA ACTIVITIES: <br />SUMMARY: <br />The CQA activities performed consisted of observing, documenting and recording the construction <br />activities of the contractor. Construction observations consisted of the removal and placement of topsoil <br />in the LOB areas. In addition, clearing and grubbing activities were observed in the LOB and the material <br />was stockpiled in the Joe Dandy slash stockpile. <br />No night shift work was performed during the week. <br />No work was performed on Saturday, January 31. Currently no work activities are planned for Saturdays <br />but Ames has indicated that Saturday will be used as a make up day in the event of delay or stoppage <br />during the current planned work periods Monday to Friday. <br />I. Testing Summary <br />No testing was performed at this time. <br />GENERAL PROJECT ITEMS <br />Summary of Concerns: <br />None noted. <br />Summary of Non-conformances: <br />None Issued <br />Summary of Meetings and Discussions: <br />CC& V.- <br />AMEC met with CCV to discuss remediation methods for the mine adits, shafts and stopes encountered <br />with the limits of the LOB delivery road cut and fill areas. AMEC visually inspected the adit located <br />beneath the fill at approximately 80 feet left of centerline near Station 37+00. AMEC concluded that no <br />remediation would be required because the adit was not located beneath a critical area and it was decided <br />that fill material can be placed directly over the opening. In addition, the Tornado shaft and an adjacent <br />stope were visually inspected; however, a remediation method was not decided at the time and additional <br />evaluations would need to be performed by A.MEC prior to making a recommendation. <br />In addition, CC&V held a Weekly Contractors' meeting on February 3, 2009. Items discussed included <br />Safety, Environmental, Schedule, QC documentation, and CQA. <br />No night shift work was performed during the week. <br />Sheet 3 of 10 LOB Weekly Report WE 02.07.09.D.doc
