III. COMMENTS -COMPLIANCE <br />Below are comments on the inspection. The comments include discussion of observations made <br />during the inspection. Comments also describe any enforcement actions taken during the inspection <br />and the facts or evidence supporting the enforcement action. <br />This was a complete inspection conducted by Kent Gorham of the Colorado Division of <br />Reclamation, Mining, and Safety (Division). This mine site is currently completing the final <br />stages of reclamation of disturbed areas. Ground conditions were dry and the weather was <br />mild for early winter. <br />Availabilitv of Records <br />The public copy of the mine records is kept at the office of Peak Project Management (PPM). <br />The contact, address, and phone number for PPM is Ron Thompson, Peak Project <br />Management, 34115 County Road 20.8, Trinidad, CO 81082, 719-846-4975. Records were <br />found in compliance as detailed on page 2a of this report. The insurance certificate expired <br />on February 8, 2009. Mr. Thompson contacted the insurance broker for a new, updated <br />certificate. <br />Hydrologic Balance <br />The weir box for ?,,• <br />z AN <br />P <br />ond 5 has now ;.tea. c ?e".'?A.%,?"??' <br />been moved to the j `? ?? rri?'; ,x :'• <br />to of Fill 8 in <br />preparation for <br />installation early this ???.: •' r.'x, ? ?' f ?-`;,,? - <br />spring. Pond 7a held <br />a small puddle of <br />water. 'w.?,i?., <br />Please also ensure <br />during the annual':. <br />pond inspection that the dewatering holes for all ponds are as represented on the design <br />Figure EX. 15-14, Sedimentation Pond Summary, of the approved permit application. <br />Pond 9 primary riser is in need of repair due to a blockage of some sort. Ponds 6, 8, 9, and <br />9a were found dry. All embankments appeared normal with no sign of damage of any kind.
