APPLICATION FORM <br />Section 3: Please fill in section 3.2 - total acreage in permit area (9.990) <br />Section 5: Section 5.2 should list the anticipated final USE of the material, not <br />anticipated final date of operation. <br />Section 12: Post mining land use selection is very important so please consider this <br />choice carefully. If you wish to change the post mining land use after the permit is issued <br />it will require a technical revision or possibly an amendment to the permit to be <br />submitted to the Division. Based on the information and reclamation plan submitted with <br />the application it appears that Rangeland (RL) may be a more suitable choice. <br />6.2 GENERAL REQUIREMENTS OF EXHIBITS <br />All maps, with the exception of the index map, must be signed and dated by the person <br />that prepared them. <br />6.3 SPECIFIC EXHIBIT REQUIREMENTS - REGULAR 110 OPERATIONS <br />6.3.1 EXHIBIT A - Legal Description <br />This section is adequate as submitted. <br />6.3.2 EXHIBIT B - Site Description <br />Although the provided index map and an air photo of the area clearly show a small pond <br />and stream immediately downgradient of the proposed permit area, these features and any <br />relevant information regarding them, are not present anywhere in the application. Please <br />describe these features, show them on the appropriate figures, and describe what <br />measures will be taken to prevent runoff from the site from impacting them (i.e. runoff <br />control, ditches, culverts etc). Simply stating that the site is porous and all runoff will be <br />directly absorbed by the site is insufficient. <br />6.3.3 EXHIBIT C - Mining Plan <br />Please show proposed roadways through the site, any structures to be placed on site, and <br />material processing areas on the mining plan map. <br />Is it anticipated that this will be an intermittent operation (i.e. periods of inactivity greater <br />than 180 days)? If so, this should be stated in the application. If this is not anticipated to <br />be an intermittent operation, then the operator will have to submit a notice of temporary <br />cessation to the Division if the mine is inactive for greater than 180 days. <br />Please specify if this operation will use explosives or not.
