• The submittal requirements for a MDP application are contained in Section 3.1). Summarized <br />below are those submittal requirements and CC&V's response: <br />1. Application Form -Attached <br />2. Specific Submittal Materials <br />a. Disclosure of Ownership - Included with Mine Life Extension application <br />to Teller County dated April 15, 2008. No information has changed. <br />b. Proof of Legal Right to Enter and Mine - Included with Mine Life <br />Extension application to Teller County dated April 15, 2008. No <br />information has changed. <br />C. Documentation of Mailed Notice - The application for change to the Mine <br />Life Extension (Amendment No. 9) approval will be submitted to the State <br />of Colorado. Based upon the determination by the Teller County Board of <br />County Commissioners that this application will be processed as a formal <br />amendment, CC&V assumes that Teller County will re-notify all owners <br />of record. All owners of mineral and surface estates as well as all adjacent <br />property owners are correctly identified in the Mine Life Extension <br />• application to Teller County dated April 15, 2008. <br />d. Adjacent Property Owners List - Included with Mine Life Extension <br />application to Teller County dated April 15, 2008. No information has <br />changed. <br />e. Mailing Labels - enclosed in this application. <br />f. Written Description - see attached written description and changed <br />sections of Project Description. <br />g. DRMS Application - A copy of the application for revision to the DRMS <br />Mine Life Extension is provided as part of this application. This <br />application to the DRMS will be submitted on or about December 19, <br />2008. <br />h. Permits or Licenses - All of the current State and/or Federal permits and <br />licenses for current operations are in place. Activities under the Mine Life <br />Extension may require modifications to those permits. The modifications <br />are not required until the covered activity is undertaken. In some cases this <br />may be several years from implementation of the Mine Life Extension. <br />Copies of new or revised permits obtained for the Mine Life Extension <br />• and this Amendment will be forwarded to Teller County when received. <br />No activities for the Mine Life Extension will be initiated until the <br />appropriate permits, approvals, or licenses are obtained.
