MINE ID # OR PROSPECTING ID #: M-2000-115 <br />INSPECTION DATE: 10/6/9008 INSPECTOR=S INITIALS: II F _ <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified <br />during this inspection. The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when <br />considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be <br />notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />r.?Tr <br />V <br /> Problem/ Possible Violation: November <br /> The proper mine entrance sign was not in place and all the permit boundary markers did 3 0th, 2008 <br /> not appear to be in place. This is a problem at this time for failure to maintain the proper <br /> mine entrance sign and boundary markers around the affected area and pursuant to Rule <br /> 3.1.12 (1-2). <br />TYPE PB <br />TOPIC SM <br />Corrective Action: <br />At the entrance of the mine site, post a sign, which shall be clearly visible from the access <br />road, with the following: the name of the operator, a statement that a reclamation permit <br />for the operation has been issued by the Colorado Mined Land Reclamation Board; and the <br />permit number. Also, place boundary markers of sufficient number and size around the <br />current approved permit boundary. Please submit photographic evidence to the Division <br />that this has been accomplished by November 14th, 2008. <br />I <br /> Problem/ Possible Violation: December <br /> A new access road appears to have been constructed into the mine site, this road is outside 3 0th, 2008 <br /> of the current permit boundary. Also about .23 acres of disturbed land appears to be <br /> outside of the eastern permit boundary. This is a possible violation at this time for failure <br /> to protect areas outside of the affected land from slides or damages occurring during the <br /> mining operation and reclamation. Pursuant to C.R.S. 34-32.5-116(4) (i) <br />TYPE PV <br />TOPIC OD, <br />RD <br />Corrective Action: <br />The operator will need to submit an amendment application to change the current <br />approved permit to include the area of the new access road and to encompass all the <br />disturbed area that is outside of the current approved permit boundary. <br />5
