The decision date for this application is October <br />12, 2008. Please be advised that if you are unable to satisfactorily address any <br />concerns identified in this review before the decision date, it will be your <br />responsibility to request an extension of the review period. If there are outstanding <br />issues that have not been adequately addressed prior to the end of the review period, <br />and no extension has been requested, the Division will deny this application. <br />1. As required by Rule 1.6.5 (2), please submit the proof of publication and proof of the <br />public notices circulated to all owners of record of surface lands, and easement <br />holders that are on the affected land and within 200 feet of the boundary of the <br />affected land. Proof of notice may be return receipts of a Certified Mailing or by <br />proof of personal service. If the Applicant has not published a public notice, then <br />please notify the Division when the public notice will be published so that Division's <br />decision due date can be extended accordingly. <br />2. The 110 Construction Materials Reclamation Permit Application that was submitted <br />to the Division was missing items 12, 13, and 14 from page 4. Please complete items <br />12, 13, and 14 on the enclosed form and submit it to the Division. <br />3. During the pre-operation inspection conducted on September 23, 2008, the inspector <br />noted that a haul road would need to be constructed to connect the proposed permit <br />boundary to the existing two-track road that leads to County Road FF. This 580 foot <br />gap between the permit boundary and the two-track road will need to be included in <br />the affected acreage.
