(Page 3) <br />MINE ID # OR PROSPECTING ID # M-2000-103 <br />INSPECTION DATE 9/5/08 <br />INSPECTOR'S INITIALS RCO <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this <br />inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible <br />violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation <br />Board. The inspector noted on the previous page should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />There were no permit ID signs or boundary markers noted, though they are required <br />pursuant to Rule 3.1.12. <br />CORRECTIVE ACTION: The operator must post an ID sign (containing all the <br />information described in Rule 3.1.12) and install durable and visible boundary markers. <br />Evidence of performing these actions must be received at this office by the date shown <br />at right. <br />2. The site was not excavated in a manner consistent with the approved mining plan, and <br />now cannot be reclaimed according to the approved reclamation plan. <br />CORRECTIVE ACTION: The operator must submit a formal permit revision or <br />amendment addressing the changes needed to adequately reclaim the site. This <br />application must be received at this office by the date shown at right. <br />3. There is a patch of Canada thistle onsite, and no formal plan in the permit for controlling <br />them. <br />CORRECTIVE ACTION: The operator must revise the permit to include an adequate <br />noxious weed control plan, and begin to aggressively treat them. (The written plan may <br />be submitted by itself or included as part of another permit modification, as in problem <br />#2, above. On-the-ground treatment should begin as soon as possible and appropriate, <br />and may precede the submittal of the written plan.)
