Seismographic records as approved in AM-08. <br />Response. <br />CC&V will review and revise the current form being used to document the production blasts <br />at CC&V to incorporate these recommendation. Should the form need to be revised, it will be <br />included forwarded to DRMS. The License number through the BATF for the site naming <br />CC&V as the License holder is: 5-CO-119-33-1B-00477 and is valid until 2011. <br />e) CC&V will conduct pre-blast survey of potentially affected structures not owned <br />or controlled by CC&V that have not previously been surveyed, with survey <br />results on file with DRMS. Pre-blast surveys must be conducted at all structures <br />located within one-half mile of the proposed blasting unless persons in ownership <br />or control deny access to the structures. Written request to conduct pre-blast <br />is surveys must be sent to owners of all structures a minimum of 30-days prior to <br />initiation of blasting within one-half mile. CC&V must provide a plan for DRMS <br />review and approval prior to conducting the surveys. <br />Response: <br />Pre- blast surveys have already been completed for the Main Cresson, and East Cresson. Pre- <br />blast surveys will be provided as a service to those within 2500 feet prior to the development <br />of the North Cresson mine. Pre-blast survey notices will be sent to those owners with copies <br />of the notices forwarded to DBMS. These notices will include a Plan completed by Matheson <br />Mining (or other qualified entity) and approved by DBMS prior to completing the surveys. <br />f) CC&V proposes the installation of at least one additional seismograph, in addition <br />to those already approved under AM-08, for monitoring of potential impacts to <br />structures in Cripple Creek. The Division will require that the location for the new <br />seismograph(s) be established prior to a decision on AM-09, and that the location <br />for one of the seismograph(s) be at the closest structure (to include the <br />underground workings of the Molly Kathleen). <br />Response: <br />Two additional production blast monitoring location near Cripple Creek would be established <br />as part of the MLE Project activities. Once of the monitors would be established in Poverty <br />• Gulch with the cooperation of the owners of Mollie Kathleen facility and the second would be <br />established based on the recommendation of Matheson Mining, at the nearest Casino in <br />Cripple Creek with the cooperation of the Casino owner. This Casino monitor would be very <br />near the Double Eagle Hotel and Casino on the corner of Myers and 5th. <br />1'1
