a <br />0 <br />Evergreen Analytical, Inc. <br />Client Project ID: <br />Lab Order: 08-4900 <br />Date: 22-Aug-08 <br />CASE NARRATIVE <br />SAMPLE RECEIVING <br />' Sample(s) were hand delivered to the laboratory by the client. <br />Custody seals were not present. <br />The temperature of the sample(s) upon arrival was 4.0 °C. <br />' Sample(s) were received in good condition, in the proper container, and within holding time. <br />Sample(s) were preserved properly. JD <br />QUALITY ASSURANCE (QA) <br />Analyses performed on samples in this work order by EAL meet the requirements of the EAL Quality <br />Assurance Program unless otherwise explained. Analyses of discharge samples meet the requirements <br />of 40 CFR Part 136 unless otherwise explained. TP <br />CLIENT SERVICES <br />The client did not provide complete contact information on the chain of custody ; the report will be <br />mailed per the quotation. The samples were logged in for TSS, Alkalinity, and pH analysis per the <br />quotation. <br />The metals and anion analytes were selected per the quotation. There are no other anomalies to report. <br />EKHJTP <br />GENERAL CHEMISTRY <br />The Anion samples had high conductivity and needed diluting. This raised the reporting limits for <br />Nitrate-N and Nitrite-N. BP <br />The pH on "DUP-0 1" is flagged "H" (out of holding time) because no sample time was given and LIMS <br />defaulted to 0000 hours. <br />There are no other anomalies to report. BP/TP <br />OIL AND GREASE/TRPH ANALYSIS <br />Method 1664 W: The batch matrix spike (on another client's sample) has a recovery below the QC <br />' limit due to matrix interference. The recovery for the laboratory control spikes (LCS/LCSD) are within <br />QC limits, proving the analysis is in control. There are no other anomalies to report. RPM/JT <br />METALS ANALYSIS <br />200.7_D: The matrix spike duplicate (MSD) recovery for calcium is above the QC limits due to the <br />' high concentration of calcium in the sample versus the low concentration of the spike.
