Sampling and Analysis Plan <br />• Field equipment calibration information <br />• Number and type of filters used <br />• Field parameters measured and results <br />Information recorded on the Surface-Water Monitoring Form will not necessarily be repeated in <br />the field log book. The field log book will refer to the sampling form when necessary. <br />5.1.3 Groundwater Monitoring Form <br />A Groundwater Monitoring Form (see Appendix B) will be used for each individual site sampled. <br />This form allows for expedited documentation of the following information: <br />• Sample identification <br />• Sample date <br />• Sample site location description and sketch <br />• Sampling personnel <br />• Equipment identification <br />• Field equipment calibration information <br />• Static water level <br />• Bore volume <br />• Total volume purged from well <br />• Measured flow rates <br />• Number and type of filters used <br />• Field parameters measured and results <br />• Other pertinent observations (gas meter readings) <br />Information recorded on the Groundwater Monitoring Form will not necessarily be repeated in <br />the field log book.
