Sampling and Analysis Plan <br />indelible ink. Each day's entries will be initialed and dated by the field sampling leader. All <br />corrections shall consist of initialed line-out deletions. <br />At a minimum, entries in the field logbook shall include: <br />• The date and time at the start of work and description of weather conditions <br />• Names of field sampling crew <br />• Project/site name <br />• Description of site conditions and any unusual circumstances <br />• Identification and locations of sample sites, sampling times, GPS Coordinates, and map <br />reference <br />• Details of actual work effort, particularly any deviations from the aforementioned <br />methods <br />• Field observations <br />• Time that field work was terminated for the day <br />• Details of photo documentation (if any) <br />Strict chain-of-custody procedures will be maintained with the field logbook. While being used <br />in the field, logbooks will remain with the field team at all times. Upon completion of the field <br />effort, logbooks will be filed in an appropriately secure manner. <br />5.1.2 Surface-Water Monitoring Form <br />A Surface-Water Monitoring Form (see Appendix B) will be used for each individual site <br />sampled. This form allows for expedited documentation of the following information: <br />• Sample identification <br />• Sample date <br />• Sample site location description and sketch <br />• Sampling personnel <br />• Equipment identification <br />American Shale Oil, LLC 10
