(Page 2). <br />MINE ID # OR PROSPECTING ID #: M-1986-061 <br />_ <br />INSPECTION DATE: - Aln.Sj11R INSPECTORS INITIALS: KAP <br />OBSERVATIONS <br />1. This inspection was conducted as part of the Division of Reclamation, Mining, and Safety's 4-year <br />monitoring program. Present at the inspection were Kate Pickford, DRMS, and Peter Kearl, Oldcastle SW <br />Group, Inc. dba Four Corners Materials. Weather at the time of the inspection was cloudy, calm, and <br />approximately 65 degrees. <br />2. Disturbance at the site consists of a large excavated area with some graded slopes, a nearly vertical <br />highwall, numerous stockpiles of material on the pit floor, and various stockpiles of topsoil. The proposed <br />mining plan indicates that the active mining face will have slopes no steeper than 1 H:1 V. This conflicts <br />with the conditions at the site. Mr. Kearl stated that Four Corners Materials will soon submit an <br />amendment to the permit to add acreage to the north end of the site. A change to the mining plan to <br />modify the maximum slope of the highwalls will be included in that amendment application. Therefore <br />this issue is not cited as a problem in this report. <br />3. Numerous weeds were observed at the site, including on some of the stockpiled topsoil. Mr. Kearl <br />stated that Four Corners has recently obtained a state-wide weed control plan and has a weed control <br />plan that is specific to this site. This information will also be submitted with the amendment application <br />for the site. Therefore this issue is not cited as a problem in this report. <br />4. Disturbance and activity at the site is encroaching on the eastern permit boundary. No boundary <br />markers were observed in this area. Therefore this issue is cited as a problem in this report. <br />5. All other conditions at the site appear to be within the guidelines of the approved application. No <br />bond calculation will be conducted at this time, as the operator will be submitting an amendment <br />application for the site in the near future. <br />I & E Contact Address Cc: ?
