Cripple Creek & Victor Gold Mining Company <br />Health and Safety Management System <br />VLF Standard Operating Procedure <br />1.0 Pad Crew members must be trained on #s 2 & 18 of the Cyanide procedures, and knowledgeable in the hazards <br />of the onsite use, and application of Cyanide here at Cripple Creek & Victor. <br />2.0 Pad Crew members must be trained in the safe operation of VLF specific motorized and electrical equipment. <br />MSHA Form 5000-23 must be issued, when Task Training Outline is complete. <br />3.0 Pad Crew members must be familiar with CC&V Environmental Guidelines. <br />4.0 VLF shall be inspected daily for operational issues - blown drip lines, leaking headers, puddling of solution, and <br />inspection of the edge of liner and edge of liner signage etc. <br />5.0 Pad Crew Supervisor on duty is responsible for insuring that the VLF alarming bypass status is appropriately <br />engaged, or disengaged, as needed for work to proceed on the pad. Engaged when not needed, and absolutely <br />engaged at the end of shift. A personal, visual, end of day confirmation of bypass engagement is required by <br />the plant operator and the acting VLF Supervisor, also to be initialed in the operators log book as to the time <br />that the Alarms were disengaged and engaged. <br />6.0 A VLF Pre-Shift Operational Safety Walk Through Form should be completed, and filed, when needed, to <br />insure a safe working area, when working in the areas indicated on the form. <br />7.0 VLF Daily Log entries will be kept current. <br />8.0 Orange cone lines established on the pad to indicate dumping/ripping limits shall not be violated by ripping <br />through, dumping over, or driving over/through. Cone lines will be placed by the VLF Supervisor (1-17) or the <br />VLF Coordinator (1-19). <br />9.0 No ripping within 20' of any active header, that is within 75' of the leach pad edge of liner. Only H-7 or H-9 <br />has the authority to grant permission, and to supervise ripping inside of this 20' no rip zone. Under no <br />circumstances will ripping within 20' of a major, active, perimeter, header happen during dark shifts. A work <br />permit is required to be filled out by the operator and signed by the VLF Supervisor before any ripping is <br />started. <br />10.0 When ripping near a perimeter header that is within 75' of leach pad edge of liner, base #4 (Channel #3) <br />must be called, and bypass engaged until ripping is completed. <br />11.0 If a major active outside perimeter header is struck, these bullet points must be followed: <br />• Call Base 4 and get the appropriate pumps and or line shut down. <br />• Isolate the break to the best of your ability by closing valves in the immediate area. <br />• Make assessment to contain all solution on the pad, calling for equipment if needed. <br />CM VLF Standard Operating Procedure Page 1 of 2 July 23, 2008 <br />Doc #: H&S 005-41 Printed copies are uncontrolled Revision 1
