MINE ID # OR PROSPECTING ID #: M-1987-L3 <br />INSPECTION DATE: X17/9942008 INSPECTOR=S INITIALS: -11.E- <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified <br />during this inspection. The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when <br />considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be <br />notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br /> Problem/Possible Violation: October let, <br />1) The boundary markers to delineate the permit boundary were not in place. This is a 2008 <br /> problem at this time for failure to maintain the proper permit boundary markers in <br /> accordance with rule 3.1.12. <br />Type Ps <br />Topic sM <br />Corrective Action: <br />The correct permit boundary will need to be delineated and marked by monuments or <br />other markers that are clearly visible and adequate to delineate such boundaries. <br />Please submit photographic evidence to the Division that the entire permit boundary has <br />been marked by October l st, 2008 <br /> <br />2.) Problem/Possible Violation: <br />The operator is backfilling the mine excavation with structural fill that has been generated <br />outside of the permit area. This is a problem at this time for failure to supply the Office <br />with the proper notification of such an activity, pursuant to Rule 3.1.5 (9) October let, <br />2008 <br />Type Ps <br />Topic BG <br />Corrective Action: <br />By October 1 st, 2008 please submit a Notice to the Office for backfilling the mine <br />excavation with structural fill that was generated outside of the permit area as described <br />in Rule 3.1.5 (9).
