f. Insurance shall include provision providing notification to the State 10 days prior to
cancellation of the coverage.
3. Automobile Liability (Minimum). The contractor must carry a minimum of$600,000
combined single limit.
VENDOR OFFSET
Pursuant to C.R.S. 24-30-202.4 (as amended), the State Controller may withhold debts owed to
state agencies under the vendor offset intercept system for: (a) unpaid child support debt or child
support arrearages; (b) unpaid balance of tax, accrued interest, or other charges specified in
Article 22, Title 39, C.R.S.; (c) unpaid loans due to the student loan division of the Department of
Higher Education; (d) owed amounts required to be paid to the unemployment compensation fund;
and (e) other unpaid debts owing to the State or any agency thereof, the amount of which is found
to be owing as a result of final agency determination or reduced to judgment as certified by the
controller.
Work Description
Maintenance work to be accomplished at the Grassy Gap Coal Mine includes construction of three
(3) low water crossings, repair of a sediment pond dewatering device and sealing of the vase of
the same sediment pond. A few miscellaneous tasks will be accomplished through the use of
machine time. Maintenance of the Grassy Gap Mine is being accomplished by the State of
Colorado, Division of Minerals and Geology, using forfeited reclamation bond monies.
The attached General Conditions for Purchase Orders and Purchase Order Terms and Conditions
apply to all work.
The Project is scheduled to be conducted during the week of June 28, 1999. It is anticipated that
the project will be accomplished in no more than four (4) consecutive working days.
The Grassy Gap Coal Mining operation was a permitted mining operation. The Colorado Mined
Land Reclamation Board has revoked the operating permit, and forfeited the reclamation bond.
The majority of the site was reclaimed by the coal mining company. The Division of Minerals and
Geology periodically performs reclamation related maintenance work at the site. Accomplishing
the required maintenance will help to stabilize the site by providing for control of snow melt and
storm related run off, and by minimizing seepage from a sediment control pond located on a
hillside.
NOTE that Mobilization and Demobilization are not included as a separate line item.
CONTRACTOR shall include charges for these items, if any, in the prices bid for the tasks
described below. No additional payment for Mobilization and Demobilization will be made.
Task 1 Low Water Crossings
The goal of this Task is to manage drainage of water on two of the of the old haul roads. In order
to accomplish this, two (2) low water crossings are to be constructed within the Project Area, one
on the Pit 4 Road, and one on the Pits 2/3 Road.
The Pit 4 Road low water crossing will be constructed immediately above the first switchback
located on this road. This crossing will serve to transport water from the north to the south side of
the road, and into an existing drainage swale. The other low water crossing will be constructed at
the first switch back of the Pits 2/3 Road. This low water crossing will allow water to drain from the
2
