6.4 SPECIFIC EXHIBIT REQUIREMENTS - REGULAR 112 OPERATIONS ' <br />6.4.1 EXHIBIT A - Legal Description <br />The Legal Description in the text is adequate, however the map submitted as A(2) should' <br />be taken from a USGS 7.5 Minute Quadrangle Map (your site is located in the northeast <br />corner of the Eckley NE Quad, but it would be helpful to include the adjacent areas of the; <br />Wauneta, Alvin SW, and Wray NW quads for reference as well, because of your site's <br />location in the far NE corner of it's quad). Also there is no datum specified for the UTM <br />coordinates you have provided as required by rule 6.4.1(2). I have included a base map <br />taken from these USGS Quad maps that includes your proposed permit area, for you to <br />modify and use where applicable. Feel free to make as many copies as you require. <br />6.4.2 EXHIBIT B - Index Map <br />All maps included with the permit application should include at a minimum the following <br />map elements: name of applicant; north arrow; scale; legend if symbols or line types are <br />used to designate map features; signature of person(s) who prepared the map; and date. <br />Permit boundaries and significant features should be clearly labeled. <br />The map you have included as Exhibit B has no scale, no legend, no indication of north, <br />and the boundary that was drawn for the permit area has two different lines that could be <br />interpreted as the northern boundary. Please carefully sketch the permit boundary on a <br />copy of the USGS Topo map for the area as described in the comments for Exhibit A. <br />6.4.3 EXHIBIT C - Pre-mining and Mining Plan Map(s) of Affected Lands <br />The map included as Exhibit C(a) is adequate to depict surrounding property owners, <br />however the rest of the required map elements (other than scale) are missing. <br />No figure has been included in your application as Exhibit C(b) to depict the features <br />described in section 6.4.3(b). If none of these features are present on or within 200 feet of <br />affected lands please state this in the application. Please be aware that all above ground <br />and buried utility lines within 200 feet of the permit area should be identified and shown. <br />The map included in the application as Exhibit C(c) has no labels on any of the sketched <br />contour lines. The easiest way to address this issue would be to use the USGS quad map <br />for this exhibit. <br />The map included as Exhibit C(d) in the application is very unclear, I believe it shows <br />only the intended permit area boundary, however, there is no information depicting the
