The following items must be observed and approved by the <br /> PROJECT MANAGER before proceeding with additional work: <br /> Placement and covering of all culverts. <br /> All final grading before the commencement of revegetation. <br /> Grading of spillways and crossings prior to riprap placement. <br /> Erosion repair prior to erosion mat installation. <br /> Broadcast seeding and tubling planting. <br /> 12. A list of attenders is attached for your information. <br /> 13. A sample "AML Contractor Ownership and Control Information Package" is also included. <br /> There is no need to return this form with your bid. The successful bidder will be required <br /> to submit this form to DMG. <br /> 14. A Bids information package is attached for your information on future bids. <br /> 15. Receipt and acceptance of this amendment shall be acknowledged on the bid form. <br /> 16. All terms and conditions of the Bid other than as changed by this amendment remain the <br /> same. <br /> 17.
