2. General Liability (Public Liability & Property Damage) (Minimum); <br /> a. Combined single limit of $600,000 written on an occurrence basis; <br /> b. Any aggregate will not'be less than $1 million; <br /> C. The contractor must purchase additional insurance if claims reduce the annual <br /> aggregate below $600,000; <br /> d. The State of Colorado must be named as additional insured on each <br /> comprehensive general liability policy; <br /> e. Certificates of insurance must be provided to the State within 10 working days <br /> after receipt of Award; and, <br /> f. Insurance shall include provision providing notification to the State 10 days prior <br /> to cancellation of the coverage. <br /> 3. Automobile Liability (Minimum). The contractor must carry a minimum of $600,000 <br /> combined single limit. <br /> Work Description <br /> Hourly bids are being solicited for work to be performed at the La Plata No. 1 mine site in Hay <br /> Gulch southwest of Durango, Colorado. Machine work will involve the recontouring of access <br /> roads to the portal area. The portal area has previously,been reclaimed but the roads will be <br /> pulled back to approximate original contour. All recontoured or otherwise impacted areas will <br /> be revegetated as described below. <br /> All equipment rental hours will be conducted per the attached 19.0 Equipment Rental <br /> specifications. The Project Manager must be on site for all equipment rental hours. The Project <br /> Manager will be available during regular business hours (8:00 A.M. to 5:00 P.M.)on weekdays. <br /> Equipment Rental time will not be scheduled on weekend days or holidays without prior <br /> approval of the Project Manager. <br /> 1) Hydraulic Excavator. Hours - Bids are being solicited for an excavator with a minimum <br /> 25'reach and a minimum 30"1 bucket with operator. Mobilization will be included in the <br /> submitted hourly rate. Work will consist of the pulling back of approximately 2,.999 14 <br /> linear feet of approximate 20' wide road. Recontouring will be to approximate original <br /> countour involving the placement of downslope material and the blending in of the upper <br /> slope cut. Payment for all machine work will be based to the nearest 1/2 hour per 19.0 <br /> Equipment Rental specifications. 40 hours are estimated to be the time required to <br /> complete the site work. This is an estimate only. The total;Alhours are NOT <br /> guaranteed. <br /> 2) Reve etation - After recontouring delineated access roads, all disturbed areas will be <br /> reseeded with seed provided by the State. Seed will be sown uniformly on the prepared <br /> areas. Broadcast seed will be covered about 1/4 to 1/2 inch deep by a method acceptable <br /> to the Project Manager. All reseeded areas will be mulched with hay or straw mulch Z� <br /> pest and weed-free. Mulch will be supplied by the Contractor. Approximately 30 <br /> percent by weight of the mulch material will be 10 inches in length or longer. Hay or <br /> straw mulch will be applied uniformly at the rate of two tons per acre.
