ORIGINAL ADD WELL CONSTRUCTION CONDITION: <br />The Permittee shall plug back the end of the we117H-2I borehole where it intersects cavity lA <br />with a 60 foot cement plug and tag the plug to ensure that the plug length is adequate. After <br />placement of an adequate plug, the borehole will be filled with water to demonstrate that the plug <br />will not leak. This information shall be submitted to the Director. See Attachment A for the <br />November 2004 Proposed Well Construction diagram for the 7H-2I well. <br />APPROVED CONSTRUCTION CHANGE: <br />In a letter, dated March 28, 2005, to Mr. Ken Phillips of our Region 8 EPA office, NSI has <br />recommended that the 7H-2I well bore and 7H cavity terminate at a location which is 251 feet <br />from the center line of the 1 A-4HI drill hole. The length of the horizontal well was reduced and <br />its placement was confirmed in a Drilling Report dated October 10, 2005. See Attachments B <br />and C for the location and revised construction diagram for the 7H-2I well. <br />Please be aware that it is the responsibility of Natural Soda, Inc., the permittee, to be <br />aware of and to comply with all requirements of the Permit.
