Documentation would clarify the discrepancies. <br />Discrepancies are found in information (Attachment 4)for Coalgrace II Inc. similar to the above for <br />Coalgrace, Inc. AVS entries consistent with the Terramatrix letter have been annotated with a check <br />mark (?). Please provide documentation for any termination of appointments listed [appointment <br />of A G Riddlesperger as secretary 10/29/1991 with concurrent (?) termination as assistant secretary <br />(appointed 05/03/1991); R B Lamm as secretary or assistant secretary (appointments of 05/30/1991 <br />and 10/29/1991; termination of R D Usilton as president with the appointment of A C Pierce; etc.). <br />Attachment 5 is an AVS printout relating to Grace Energy Corp. Those entries marked with a ? are <br />consistent with the information provided in the Terramatrix letter. If the AVS databank is correct, <br />the identification of interests section of the permit should reflect the additional appointments (please <br />confirm to me that the additional appointments are appropriate). If any of the appointments of the <br />AVS databank have been terminated, please provide me with appropriate documentation to make <br />the AVS changes. <br />Attachment 5 applies to W R Grace & Co CONN. In general, the AVS information is more <br />complete than the information provided in the Terramatrix letter. For example, the permit should <br />also reflect the appointment of Albert J Costello as Chairman of the Board in addition to his <br />appointments as President and CEO; P .D Houchin and R H Beber as directors; and Larry Ellberger <br />and G Koenzig Jr. as senior vice presidents. Please provide documentation if the appointments of <br />F P Boer as executive vice president, Fred Lempereur as senior vice president, Robert J. Bettacchi <br />as vice president, and James R Hyde as senior vice president have been terminated. If their <br />appointments have not been terminated, they should appear in the identification of interest listing <br />of the permit. The Terramatrix letter indicates, in general, that these appointments have been <br />terminated, but the information is not specific to individuals, and dates of termination are not <br />provided. <br />Attachment 6 applies to W R Grace & Co. As for W R Grace & Co CONN, the AVS information <br />is more complete than the information provided in the Terramatrix letter. If the following new <br />appointments (AVS databank) are correct, the identification of interests section of the permit should <br />reflect them: M A Fox as director, Albert J Costello as chairman of the board, and G Koenzig Jr as <br />senior vice president. <br />Please contact me if you have any questions.
