In addition to the information provided on pages 9b and 9c of the application, <br />please provide any alias these individuals may have, and their social security numbers (social <br />security numbers are not mandatory, but the use of social security numbers reduces the chances of <br />confusion with others of similar name). <br />The Division of Minerals and Geology (Division) issues a decision approving or denying a minor <br />revision application within 10 days of filing of a complete application [Rule 2.08.4(6)(ii)]. In the <br />absence of the requested information, the Division would be required to deny the application. If <br />you wish this application to remain active, please waive your right to a decision within the 10- <br />day time frame, and request the decision date be extended to a specific date.
