Coaldale Quarry. <br />Dear Mr. Lamanna: <br />Matheson Mining Consultants, Inc. (MMC) is pleased to provide Holcim US Inc. (Holcim) with <br />a proposal for preblast surveys, water well sampling and vibration monitoring services for the <br />Coaldale Quarry currently under permit #M1977247 by the Colorado Division of Reclamation <br />' Mining and Safety (DRMS). <br />MMC possesses the qualifications required for the preblast surveys required by 30 CFR 816.62 <br />Use of explosives: Preblasting survey (Appendix 11I). MMC is also qualified to conduct <br />vibration monitoring according to 30 CFR 816.67 Use of explosives: Control of Adverse Effects <br />(Appendix III) Additionally MMC will conduct water sampling at all accessible residential <br />wells located on properties associated with the preblast surveys. MMC qualifications are <br />' detailed in Appendix I of this report. ACZ Laboratories in Steamboat Springs, Colorado will <br />conduct all subsequent laboratory analysis of water samples collected by MMC. ACZ <br />Laboratories qualifications and certifications are listed in Appendix II. <br />Method <br />MMC will examine the all accessible areas of the occupied structures listed in Appendix IV. <br />Copies of the inspection reports will be given to the DRMS and to the property owners. <br />The inspection report is based on an examination of only those portions of the premises readily <br />accessible for visual inspection. No inspection will be performed in areas that are obstructed, <br />concealed, or closed off. No inspection will be performed in areas that required the dismantling, <br />removal, or movement of any object including, but not limited to: roof, wall, floor and ceiling <br />coverings, furniture and appliances. <br />The condition of the premises may change after the dates of inspection due to factors such as <br />' weather, fire, moisture, leaks, and action taken by the owner or others, or the passage of time. <br />The inspection report will describe the condition of the premises at the time of inspection, and <br />
