G.) Please specify is groundwater will be encountered and/or if surface water will be <br />intercepted, describe how mining will affect the quantity and quality of the surface or <br />groundwater, and describe the methods that will be used to minimize disturbance to the <br />surface and groundwater systems including any sediment containment systems and/or <br />storm water runoff controls. <br />H.) Please describe how you will comply with the applicable Colorado water laws and <br />regulations governing injury to existing water rights. Also, please describe what <br />measures will be taken to minimize disturbance to the hydrologic balance, prevent offsite <br />damage, and provide for stable configuration of the reclaimed area consistent with the <br />proposed future land use. <br />I.) Will the mined material be processed within the permit site? If so, please describe the <br />equipment that will be used for processing and/or any processing facilities, and any <br />chemicals that will be used for processing the material. <br />J.) Please describe the primary commodity to be mined and will any secondary <br />commodity be mined and what they will be used for. Will any incidental commodities be <br />mined, and what will they be used for. <br />6.3.4 EXHIBIT D - Reclamation Plan <br />A.) At what point will the overburden material and the topsoil be replaced in the mine <br />area? <br />B.) Please submit a detailed revegetation plan with the following information: <br />1.) When will the site be re-vegetated? <br />2.) How will the seedbed be prepared for seedling establishment? <br />3) Will any fertilizer be used? If so, what type will be used and how much will <br />be applied? Please note, soil amendments shall only be applied where soil test <br />indicate nutrient deficiencies for the plant species. <br />4.) What species of grass will be planted, and why where they chosen? <br />5.) State the amount of seeds that will be planted per acre (pounds of pure live <br />seed per acre). <br />6.) Will mulch be applied to the area? If so, what kind of mulch will be used, and <br />how much will you apply per acre? Will you crimp the mulch in place? <br />6.3.5 EXHIBIT E - May <br />D.) The office received three maps for this section, "Exhibit E" which displays the <br />surrounding land owners, "Exhibit E-Phase 1" and "Exhibit E-Phase 2" which depicts the <br />mining operation and briefly describes the reclamation plan. On the "Exhibit E-Phase 1" <br />and "Exhibit E-Phase 2" maps, please indicate the direction the material extraction will <br />proceed. Also, if you will be placing any storm water diversions or impoundments on the <br />site, please display them on these maps. Please label any existing disturbances within
