-ii- <br />FILI`~IG REQUIREMENTS: <br />In order t~ apply for a Reclamation Permit for a Regular 112 Operation, please provide: <br />° One (1) signed and notarizEd completed ORIGINAL and one (1 } Dopy of the completed original Regular t 12 <br />Operation Application Form. ORIGINAL SIGNATURES MUST BE DONE IN BLUE INK. <br />_,____ ° Two {2) copies of Exhibits A-S (required sections described in Rufe 6). <br />° Two (2) copies of Addendum 1 -Notice requirements (described in Rule 1.6.2(1 Kb}). A sample of this notice is <br />attached for your use. <br />° The t,eoteehnical Stability Exhibit when required by the Division. <br />° The applicaton fee. <br />The ninety (90) day period for review of the application and exhibits will NOT begin until all required information and fee are <br />submitted. The O#fice will then review the submitted information far adequacy. <br />NOTICE .REQUIREMENTS: <br />_~ 1. You MUST send a notice, on a form approved by the Board, to the local board of county commissioners. A <br />copy of this "Notice of Filing Application" farm is attached for your use. <br />_ 2. If the Honing operation is within the boundaries of a conservation district, send a notice to the board of <br />supervisors of the conservation district, PRIOR to filing the application. A copy of this "hlotice of Filing <br />Application" form is attached for your use. <br />3. You MUST include proof of notice #1 and #2 above with the application at the time the application is <br />submitted to the Office for filing (Rule 1.6.2(2){g}}. <br />4. PRIOR to filing the application, place for public review a copy of the application, less confidential items, with <br />the clerk or recorder of the county or counties in which the affected land is Iocated. <br />5. You MIDST include an affidact or receipt demonstrating that the application was filed with the county clerk or <br />recorder at the time the application is submitted to the Office far filing. <br />6, Any changes or additions made to an application submittal MAST be filed with the county ciexk ar recorder. <br />You MtTST also provide the Office with an affidavit ar receipt demonstrating that the change was tiled with the <br />county clerk or reLOrder no later than the close of business an the day the change was filed with the Office <br />{Rule 1.8.1{2}}, <br />7. Within ten (1 U) days after your application is considered filed, you must publish four tunes in a newspaper of <br />general circularion, in the locality of the proposed mining operation, the notice described in Rule 1.6.2{t}(d}. <br />8. In addition, after the first publication you must mail ar personally serve a copy df the notice described in <br />Rule 1.6.2{ixd) to all owners of record of surface rights to the affected land and all owners ofrecard of lands that <br />are within 2a(} fezt of the boundar3r of the affected laud {R.ule l .ft.2(l.}{e)). A copy of a form which includes all <br />required information for the notice is attaached far your use. <br /> <br />
