EXHIBIT M - Other Permits and Licenses <br />1. Moffat County Conditional (Special) Use Permit (Currently in process) <br />2. General Permit for Stormwater Discharges Associated with Sand and Gravel <br />Operations and SWMP. <br />3. APEN (air pollution permit) An Air Pollutant Emission Notice (APEN) application <br />has been filed with the CDPHE - Air Pollution Control Division; other permits are to <br />be current with any mobile material processing equipment ( see 8 below; for crushing, <br />screening and asphalt plant operations) to be used at the site. The APEN will be <br />revised as necessary to reflect actual site annual production volumes. <br />4. CDOT - R.O.W. Access Permit Application - for access to Highway 394. <br />5. State Of Colorado, Division of Water Resources - Gravel Pit Well Permit <br />Applications. Application form for each reclaimed pond water surface. <br />6. U.S. Army Corps of Engineers Notification Letter and Request for Comments; <br />USACE Grand Junction, Colorado Office. At this time the USACE has not provided <br />any response or comments to the applicant. <br />7. A Notification Letter was sent to N.W. Regional Manager for The Colorado Division <br />of Wildlife, Grand Junction, Colorado office requesting comments. Copies of the <br />letter and DOW response are attached to Exhibit H. <br />8. All temporary mobile processing equipment for sand, gravel and asphalt production <br />operations will have all permits as required by the State of Colorado. These permits <br />apply to each equipment type to be moved onto and operated at the site. Copies of all <br />such permits and records will be made available to the County.
