o If the permittee intends to convert the well to a monitoring well, a plan for <br />the recompletion of the well as a monitoring well shall be submitted to the <br />Director at the same time that the permittee notifies the Director of its <br />intent to convert the production well to a monitoring well. <br />o The permittee shall either complete the conversion of the production well <br />to a monitoring well or plug the well within 60 days of the date of the <br />conversion notice, unless otherwise approved by the Director. <br />o Water level monitoring in any converted dissolution aquifer monitoring <br />wells shall be as specified for dissolution surface monitoring wells in the <br />Water Monitoring Plan (Appendix I). <br />If water-level monitoring indicates that the Injection Zone outside of the Injection <br />Interval is being pressurized, corrective action is required as specified in Part II D <br />(1) (k) of this Permit. - <br />*** <br />6. Injection of any hazardous waste as identified by EPA under 40 CFR § 261.3 <br />is prohibited. U op n any major change in the fluid sources the Permittee must re- <br />demonstrate compliance with 40 CFR & 262.11. The Water Monitoring Plan <br />(Appendix I) also includes this requirement <br />*** <br />D. MONITORING, RECORD KEEPING, AND REPORTING OF <br />RESULTS <br />MONITORING: <br />(ls` Paragraph) The permittee is required to cam out the activities described in the <br />monitoring elan The data must be reported and maintained as provided in Section II D.4 <br />of the Permit The Ground Water Surface Water and Process Monitoring Plan (Water <br />Monitoring Plan), dated September 2004, has been incorporated into the Permit as <br />Appendix I <br />(2nd paragraph) The Water Monitoring Plan includes ground-water monitoring, for the <br />Piceance Site (solution well field and upper plant) and process monitoring for mining <br />operations at the Piceance Site. It also contains monitoring provisions for surface ponds <br />and the pipeline, which are not covered by the Permit. <br />*** <br />(4`'' paragraph, modified 5`'' paragraph replaces deleted 4`'' paragraph) The Water <br />Monitoring Plan addresses monitoring for the current mining area (0-5 year panel). <br />Subsequent monitoring plans, applicable to the future commercial mining areas (6-30 <br />year panels), will be developed prior to mining in those areas.
