Rule 6.3.3 Exhibit C~- Mining Plan <br />4. The applicant should commit, in writing, to staying two (2) feet above the seasonal high <br />ground water elevation unless it can be demonstrated the mining will not intercept ground <br />water at a lesser depth. <br />5. Please designate if disturbances such as benches or impoundments (possibly used to hold <br />water on site) will occur, and if so specify their dimensions. <br />6. Please affirm the quantity of topsoil that will be replaced during reclamation. <br />Rule 6.3.4 Exhibit D-Reclamation Plan <br />7. Please describe the practices, structures, and/or measures to be implemented to control <br />sedimentation and erosion from the mine site into the nearby Smoky Hill River drainage. <br />Rule 6.3.5 Exhibit E-- Map <br />8. Please submit a map labeling the names of the owners of record of the surface of the <br />affected area and of the land within two hundred (200) feet of the affected area per Rule <br />6.3.5(2)(b). <br />9. Please provide anticipated quantity (cubic yards) of topsoil on-site at all times. <br />10. Please provide anticipated quantity (cubic yards) of overburden on-site at all times. <br />Rule 6.3.9 Exhibit I -- Proof of Filing with Count <br />11. Please provide an affidavit or receipt (signed by the County Clerk and Recorder) <br />indicating the date on which the application was placed with the County Clerk and <br />Recorder for :public review. <br />Rule 6.3.10 Exhibit J -Proof of Mailing Notices to Board of County Commissioners and Soil <br />Conservation District <br />12. Please provide proof (in the form of Certified Mail receipts, a signed affidavit, or <br />signatures on the notices themselves) that these notices of the permit application were <br />received by the Board of County Commissioners and Soil Conservation District. <br />Rule 6.3.12 Exhibit I~ Permanent Man-Made Structures <br />13. Please definitively state whether or not any permanent man-made structures are located <br />within the proposed permit boundaries, or within 200 feet of the proposed permit <br />boundary. <br />14.
