Cattany <br />Division Director <br />Natural Resource Trustee <br />Listed below are the additional items and information we are requesting in order to complete our <br />adequacy review on Ithe Verhoeff Pit #2, New 110(c) Application, Permit No. M-2008-013. The <br />Division of Reclamation, Mining, and Safety (Division) is required to make an approval or <br />denial decision no later than April 25, 2008, therefore, your response to the following adequacy <br />review concerns shouuld be submitted to the Division as soon as possible. Your response should <br />include two (2) copies. Any public comments that we receive on your application during the <br />interim will be forwarded to you. <br />Please submit the proof of notice owners of record of all land surface within 200 feet of <br />the boundary of the affected lands. These owners of record were not disclosed in the <br />application (see issue #8 below). If the applicant is unable to provide signed certified <br />mail receipts of said notice, then proof of the applicant's good faith attempt to provide <br />notice is required. Attempts to deliver certified mail can be obtained from the US Postal <br />Service in lieu of signed receipt certification. <br />2. Rule 6.2.1(2) requires that maps be signed by a registered land surveyor, professional <br />engineer, or c-ther qualified person (this "other qualified person" has been the operator in <br />other past applications). The maps submitted to the Division were not signed. Please <br />resubmit signed copies of the maps. <br />Rule 6.3.2 Exhibit B-• Site Description <br />3. Per Rule 6.3.2(a) please provide a "description of the vegetation and soil characteristics <br />in the area oJF the proposed excavation."
