(Page 2)
MINE ID # OR PROSPECTING ID # M-1993-059
INSPECTION DATE 10/26/2007 INSPECTOR'S INITIALS DEZ
OBSERVATIONS
This inspection of Permit Number M-1993-059 (Midwestern Farms Resource) was conducted by Deb E. Zack of the DRMS as
part of the Division's routine monitoring protocol. Kirkland Construction representative Joe Baxter was available for this
inspection. The pit is located less than 2 miles south and west of Holly, CO and currently consists of approximately 1479.20
acres, of which approximately 117 acres appear to be involved in active mining operations.
The mine entrance sign and permit boundaries were all clearly marked. The site was active at the time of this inspection.
Topsoil is stockpiled at the northern end of the property. The elevated conveyor that is set up crosses the railroad tracks,
bringing material from the southern portion of the permit area (the active excavation area) to the haul area. Processing and
stockpiling appear to occur mainly on the portion of the site that lays to the south of the railroad tracks.
At the present time, the Division does not have a copy of the current approved Substitute Water Supply Plan on file with the
Office of the State Engineer. The most recent submission the Division has of this document expired on April 30, 2007
(enclosed). Therefore, the Division requires a copy of the current Substitute Water Supply Plan, on file with the Office of the
State Engineer, to be submitted to the Division on or before March 31, 2008 (this is listed as Problem No. 1 on page 3).
In addition, during the scheduling of this inspection, difficulties arose in finding a current point of contact for this site (this is
listed as Problem No. 2 on page 3). Therefore, to resolve this issue, the Division requests alist of up-to-date points of contact
on or before March 31, 2008 (including the names, addresses, and phone numbers) for the person or persons most likely to:
1. Accompany an inspector (inspection contact)
2. Answer questions regarding the status of current operations (operator contact)
3. And the person to whom requests for further information or documents should be directed (permitting contact)
All other activities and completed work at the time of inspection appeared to follow the guidelines and regulations set forth by
the state and in the mining and reclamation plans for the pit.
Responses to this inspection report should be directed to Deb E. Zack at the Division of Reclamation, Missing and Safety,
Denver Office, 1313 Sherman Street, Room 215, Denver, Colorado 80203, phone number 303-866-4179.
I & E Contact Address
NAME
OPERATOR
STREET
CITY/STATE/ZIP
Mr. Joe C.
