(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1993-059 <br />INSPECTION DATE 10/26/2007 INSPECTOR'S INITIALS DEZ <br />OBSERVATIONS <br />This inspection of Permit Number M-1993-059 (Midwestern Farms Resource) was conducted by Deb E. Zack of the DRMS as <br />part of the Division's routine monitoring protocol. Kirkland Construction representative Joe Baxter was available for this <br />inspection. The pit is located less than 2 miles south and west of Holly, CO and currently consists of approximately 1479.20 <br />acres, of which approximately 117 acres appear to be involved in active mining operations. <br />The mine entrance sign and permit boundaries were all clearly marked. The site was active at the time of this inspection. <br />Topsoil is stockpiled at the northern end of the property. The elevated conveyor that is set up crosses the railroad tracks, <br />bringing material from the southern portion of the permit area (the active excavation area) to the haul area. Processing and <br />stockpiling appear to occur mainly on the portion of the site that lays to the south of the railroad tracks. <br />At the present time, the Division does not have a copy of the current approved Substitute Water Supply Plan on file with the <br />Office of the State Engineer. The most recent submission the Division has of this document expired on April 30, 2007 <br />(enclosed). Therefore, the Division requires a copy of the current Substitute Water Supply Plan, on file with the Office of the <br />State Engineer, to be submitted to the Division on or before March 31, 2008 (this is listed as Problem No. 1 on page 3). <br />In addition, during the scheduling of this inspection, difficulties arose in finding a current point of contact for this site (this is <br />listed as Problem No. 2 on page 3). Therefore, to resolve this issue, the Division requests alist of up-to-date points of contact <br />on or before March 31, 2008 (including the names, addresses, and phone numbers) for the person or persons most likely to: <br />1. Accompany an inspector (inspection contact) <br />2. Answer questions regarding the status of current operations (operator contact) <br />3. And the person to whom requests for further information or documents should be directed (permitting contact) <br />All other activities and completed work at the time of inspection appeared to follow the guidelines and regulations set forth by <br />the state and in the mining and reclamation plans for the pit. <br />Responses to this inspection report should be directed to Deb E.
